By submitting payment information, you authorize Modugno Family Dental to charge the payment method provided for the amount specified at the time of payment.

You agree to provide accurate and complete payment information. You are responsible for ensuring that your payment details are current and valid.

Online payments may be applied to outstanding balances, treatment fees, insurance-related patient responsibilities, deposits, or other charges associated with services provided by the Practice.
